Thermo-fluidic devices and materials inspired from mass and energy transport phenomena in biological system

摘要: Mass and energy transport consists of one of the most significant physiological processes in nature, which guarantees many amazing biological phenomena and activities. Borrowing such idea, many state-of-the-art thermo-fluidic devices and materials such as artificial kidneys, carrier erythrocyte, blood substitutes and so on have been successfully invented. Besides, new emerging technologies are still being developed. This paper is dedicated to presenting a relatively complete review of the typical devices and materials in clinical use inspired by biological mass and energy transport mechanisms. Particularly, these artificial thermo-fluidic devices and materials will be categorized into organ transplantation, drug delivery, nutrient transport, micro operation, and power supply. Potential approaches for innovating conventional technologies were discussed, corresponding biological phenomena and physical mechanisms were interpreted, future promising mass-and-energy-transport-based bionic devices were suggested, and prospects along this direction were pointed out. It is expected that many artificial devices based on biological mass and energy transport principle will appear to better improve various fields related to human life in the near future.

摘要:

科学的监管体系可以促进新兴事物的蓬勃和规范化发展。人工智能(AI)独立医用软件是人工智能时代赋能医疗健康领域的重要产物。随着人工智能的深入发展,人工智能独有的黑盒算法及自主学习能力引起了巨大的监管挑战。AI 独立医用软件的监管需要与时俱进,为最大程度降低人工智能医疗软件不良事件发生率和风险影响,我们亟待寻求更为科学合理的监管应对方案。对此,本文从AI 技术特征监管应对出发,针对算法变更管理、质量控制、安全追溯等监管上存在的痛点和难点问题梳理了国内外AI 独立医用软件相关监管制度和支撑资源的现状。分析总结了我国AI 独立医用软件监管在制度层面、支撑资源层面仍面临的一些问题与挑战,并针对上市后监管短板提出了措施建议:系统完善AI 独立医用软件监管制度,深化AI 独立医用软件监管支撑体系。以期能够为进一步完善我国AI 独立医用软件科学监管提供参考。

Ingestible wireless capsules for enhanced diagnostic inspection of gastrointestinal tract

Wireless capsule endoscopy has become a common procedure for diagnostic inspection of gastrointestinal tract. This method offers a less-invasive alternative to traditional endoscopy by eliminating uncomfortable procedures of the traditional endoscopy. Moreover, it provides the opportunity for exploring inaccessible areas of the small intestine. Current capsule endoscopes, however, move by peristalsis and are not capable of detailed and on-demand inspection of desired locations. Here, we propose and develop two wireless endoscopes with maneuverable vision systems to enhance diagnosis of gastrointestinal disorders. The vision systems in these capsules are equipped with mechanical actuators to adjust the position of the camera. This may help to cover larger areas of the digestive tract and investigate desired locations. The preliminary experimental results showed that the developed platform could successfully communicate with the external control unit via human body and adjust the position of camera to limited degrees.

The recent advances in integrated circuit technology, wireless communication, and sensor technology have opened the door for development of miniature medical devices that can be used for enhanced monitoring and treatment of medical conditions. Wireless capsule endoscopy is one of such medical devices that has gained significant attention during the past few years. It is envisaged that future wireless capsule endoscopies replace traditional endoscopy procedures by providing advanced functionalities such as active locomotion, body fluid/tissue sampling, and drug delivery. Development of energy-efficient miniaturized actuation mechanisms is a key step toward achieving this goal. Here, we review some of the actuators that could be integrated into future wireless capsules and discuss the existing challenges.

With the advancement in micro- and nanotechnology, electromechanical components and systems are getting smaller and smaller and gradually can be applied to the human as portable, mobile and even wearable devices. Healthcare industry have started to benefit from this technology trend by providing more and more miniature biomedical devices for personalized medical treatments in order to obtain better and more accurate outcome. This article introduces some recent development in non-intrusive and intrusive biomedical devices resulted from the advancement of niche miniature sensors and actuators, namely, wearable biomedical sensors, wearable haptic devices, and ingestible medical capsules. The development of these devices requires carful integration of knowledge and people from many different disciplines like medicine, electronics, mechanics, and design. Furthermore, designing affordable devices and systems to benefit all mankind is a great challenge ahead. The multi-disciplinary nature of the R&D effort in this area provides a new perspective for the future mechanical engineers.

Efficient detection methods for amplify-and-forward relay-aided device-to-device systems with full-rate Article

摘要: 中继辅助设备到设备通信是下一代蜂窝网络中一项极具潜力的技术。研究了适用于配置多天线的放大转发中继辅助设备到设备通信系统的不同传输方案。为了解决基于全速率空时块码传输方案的传统最大似然检测算法复杂度高的问题,文中提出两种低复杂度检测方法,分别为采用最大似然合并算法的检测方法和采用联合条件最大似然检测器的检测方法。特别是,所提的基于联合条件最大似然检测器的检测方法,能够通过牺牲一定的存储空间换取较好的并行处理能力。仿真结果表明:针对采用全速率空时块码传输方案提出的两种检测方法,能够获得与传统最大似然检测方法近乎相同的误符号率,但所提检测方法复杂度更低;同时,采用基于全速率空时块码的传输方案比复用传输方案具有更优的误码性能。仿真结果还进一步验证了对所提检测方法的分集增益分析。

A literature review of perishable medical resource management

摘要: In recent decades, healthcare providers have faced mounting pressure to effectively manage highly perishable and limited medical resources. This article offers a comprehensive review of supply chain management pertaining to such resources, which include transplantable organs and healthcare products. The review encompasses 93 publications from 1990 to 2022, illustrating a discernible upward trajectory in annual publications. The surveyed literature is categorized into three levels: Strategic, tactical, and operational. Key problem attributes and methodologies are analyzed through the assessment of pertinent publications for each problem level. Furthermore, research on service innovation, decision analytics, and supply chain resilience elucidates potential areas for future research.

Design method and verification of a hybrid prosthetic mechanism with energy-damper clutchable device

摘要: Transfemoral amputees (TAs) have difficulty in mobility during walking, such as restricted movement of lower extremity and body instability, yet few transfemoral prostheses have explored human-like multiple motion characteristics by simple structures to fit the kinesiology, biomechanics, and stability of human lower extremity. In this work, the configurations of transfemoral prosthetic mechanism are synthesized in terms of human lower-extremity kinesiology. A hybrid transfemoral prosthetic (HTP) mechanism with multigait functions is proposed to recover the gait functions of TAs. The kinematic and mechanical performances of the designed parallel mechanism are analyzed to verify their feasibility in transfemoral prosthetic mechanism. Inspired by motion–energy coupling relationship of the knee, a wearable energy-damper clutched device that can provide energy in knee stance flexion to facilitate the leg off from the ground and can impede the leg’s swing velocity for the next stance phase is proposed. Its co-operation with the springs in the prismatic pairs enables the prosthetic mechanism to have the energy recycling ability under the gait rhythm of the knee joint. Results demonstrate that the designed HTP mechanism can replace the motion functions of the knee and ankle to realize its multimode gait and effectively decrease the peak power of actuators from 94.74 to 137.05 W while maintaining a good mechanical adaptive stability.

Effectiveness of preventive medicine education and its determinants among medical students in Malaysia

Preventive medicine has been incorporated in the medical school curriculum, but its effectiveness and the factors that affect it are yet to be widely looked into in the context of Malaysia. We aimed to measure the familiarity with, perception about the importance to learn, and the ability to practice preventive medicine as well as its determinants among the medical students in Malaysia. Thus, a cross sectional study was conducted through an anonymous online survey among 387 randomly selected final year medical students of four large public medical schools in Malaysia from March to September 2014. Of the total sample, only 340 (response rate 87.8%) gave a written informed consent and took part in the survey. The familiarity of the sample with preventive medicine was measured in 19 preventive medicine areas, and their perception about the importance of preventive medicine and their ability to practice it were gauged on a Likert scale (low score indicates disagreement and high indicates agreement). Descriptive statistical analysis was performed, followed by logistic regression. The mean age of the respondents was 23.7 (SD 0.77) years, and 61.2% (n = 208) of them were females. Results showed that 22.9% of the sample (n= 78) had a low familiarity with preventive medicine, whereas 76.8% (n = 261) had a high familiarity. The study sample specified that among all the preventive medicine subjects, screening and control as well as smoking cessation and immunization are “extremely important to learn.” In univariable analysis, being a female, medical school, family size, and perception about the importance to learn preventive medicine were associated with the ability to practice it. In multivariable analysis, the perception towards the importance to learn preventive medicine was the only significant determinant: aOR (adjusted odds ratio) for those who “agreed” 17.28 (95% CI aOR 4.44 – 67.26, P<0.001) and for “strongly agreed” 35.87 (95% CI aOR 8.04 – 159.87, P<0.001). Considering these findings, the familiarity of medical students with preventive medicine should be increased. The perception about the importance to learn preventive medicine is a strong determinant for the ability to practice it.

Experiment and optimal design of a collection device for a residual plastic film baler

摘要: It is imperative to carry out research on residual plastic film collection technology to solve the serious problem of farmland pollution. The residual plastic film baler was designed as a package for film strip collection, cleaning and baling. The collection device is a core component of the baler. Response surface analysis was used in this study to optimize the structure and working parameters for improving the collection efficiency of residual film and the impurity of film package. The results show that the factors affecting the collection rate of residual film and the impurity of the film package are the speed ratio ( ) between the trash removal roller and eccentric collection mechanism, the number ( ) and the mounting angle ( ) of spring teeth in the same revolution plane. For the collection rate, the importance of the three factors are in the order, . Meanwhile, for the impurity, the importance of three factors are in the order, . When the speed ratio, the mounting angle and the number of spring teeth was set at 1.6°, 45°, and 8°, respectively, the collection rate of residual film was 88.9% and the impurity of residual film package was 14.2% for the baler.

Attitude toward preventive counseling and healthy practices among medical students at a Colombian university

Unhealthy behaviors of medical students influence their attitudes toward preventive counseling. The burden of chronic diseases is continually growing in developing countries, emphasizing the need for the increased role of general practitioners in preventive counseling. The objective of this study was to describe the effect of medical training on the risk profiles and attitudes of medical students toward preventive counseling in a Colombian university. Students in their first and fifth years of training were surveyed using the “Healthy Doctor= Healthy Patient” questionnaire to assess counseling attitudes; personal practices, such as risky drinking, smoking, inadequate nutrition, and non-compliance with physical activity recommendations; and the university environment. The association among these components was estimated, as well as the effect of the university environment. Risky drinking and smoking decreased from first to fifth year (59.3% vs. 37.1%, P = 0.021 and 31.5% vs. 25.9%, P = 0.51, respectively), whereas inadequate nutrition and non-compliance with physical activity recommendations increased. Physical activity (PA) was associated with positive counseling attitude (ORs: nutrition 7.6; alcohol 5.2; PA 10.6). Areas governed by institutional policies that are emphasized in the curriculum positively affected student practices. PA promoted preventive counseling and healthy lifestyles most effectively. Universities should therefore strengthen their preventive medicine curricula and modify social determinants.

在过去的几十年间,增材制造(additive manufacturing, AM)技术可低成本、高效益地制造几何外形复杂的物体,在医疗行业得到了快速的发展和广泛的应用。在本文中,我们讨论了AM技术在制药学、医用植入物和医疗器械领域中的最新应用进展。口服和透皮给药的药物可以通过多种AM技术制备,不同类型的软硬医用植入物也可通过AM技术实现组织工程结构的制作。此外,利用AM技术还发明了用于诊断和治疗各种病理情况的医疗器械。越来越多的研究揭示了AM技术在医疗应用中的巨大潜力。本文目的是阐述AM技术在医学领域的应用进展以及目前的局限性。

Methodological challenges to human medical study

With the transformation of modern medicinal pattern, medical studies are confronted with methodological challenges. By analyzing two methodologies existing in the study of physical matter system and information system, the article points out that traditional Chinese medicine (TCM), especially the treatment based on syndrome differentiation, embodies information conception of methodological positions, while western medicine represents matter conception of methodological positions. It proposes a new way of thinking about combination of TCM and western medicine by combinating two kinds of methodological methods.
